Getting Started
Most patients are either referred to a specialist at MSHS or need to meet with one of our providers for an initial examination to determine the best course of future care.
If you already have an appointment scheduled you can fill out the new patient forms, if you need to schedule an appointment you may contact a doctor office directly or contact our main office at (906) 643-8585 to schedule.
New Patient Documents
If you wish, you can securely fill out our new patient documents and sign them online. We’ll then have them securely, electronically documented and ahead of your appointment to save you time.
Before your office visit
If you have been given any fasting or special instructions by your physician please follow those before your appointment.
If you need to cancel or reschedule
Please contact us at least 24 hours before your appointment.
